Neuro Independence ("we", "us", "our") provides mobile neurological physiotherapy services across the Hibiscus Coast and surrounding areas.  This Privacy Policy explains what personal and health information we collect, why we collect it, how we use, store, and disclose it, and what rights you have. It applies to information voluntarily provided through our website, over the phone, by email, and during in-person consultations and treatment. This information may include your name, address, phone number, and other details you provide.

This policy is written to comply with the Privacy Act 2020 and the Health Information Privacy Code 2020 (HIPC), which together set out how health agencies in New Zealand must handle personal health information.

We collect and hold your information to:

●      Assess your condition and provide safe, appropriate physiotherapy treatment

●      Develop and adapt your personalised treatment plan

●      Communicate with you about appointments, your care, and your account

●      Process ACC claims and liaise with ACC where you are receiving ACC-funded treatment

●      Coordinate care with other health professionals involved in your treatment, with your knowledge

●      Meet our legal, professional, and regulatory obligations (including those set by the Physiotherapy Board of New Zealand)

●      Improve our services

Storage and Security

We take reasonable steps to protect your information from loss, unauthorised access, use, modification, or disclosure, in line with Rule 5 of the HIPC. This includes:

●      Storing clinical records in “Splose” (our practice management software), which uses encrypted, access-controlled cloud storage

●      Using strong, unique passwords and two-factor authentication to access client records

●      Limiting access to your information to those directly involved in your care or the running of the practice

●      Securely disposing of information once it is no longer required to be held.

Your Right to Access and Correct Your Information

You have the right to:

●      Ask us to confirm what personal and health information we hold about you

●      Request a copy of that information

●      Ask us to correct any information you believe is inaccurate, incomplete, or out of date

To make a request, email info@neuroindependence.co.nz. We will respond within the timeframes required by the Privacy Act 2020 (generally as soon as reasonably practicable, and no later than 20 working days). In most cases, there's no charge for requesting access to your own information.

When you submit an enquiry through our website contact form, the information you provide (name, contact details, and any details about your enquiry, including ACC status) is sent to us and handled in line with this policy.
